Keratoconus is a condition that affects both eyes, often undiagnosed because it starts with low amounts of astigmatism that can often be corrected with regular eyeglasses and contact lenses. A routine eye examination will usually miss the very early signs of Keratoconus unless a special scan is performed. Previous studies have shown that 5 in 10000 (1 in 2000) people have Keratoconus. A new study by the American Academy of Ophthalmology suggests that this prevalence is much higher, at 1.38 per 1000 people.

 

The earlier the diagnosis, the more important it is because the condition leads to irregular astigmatism and thinning of the cornea. As the condition worsens, regular eyeglasses or contact lenses may no longer be effective, leading to very poor vision. 

As Keratoconus progresses, the cornea can bulge forward and, in the process, thin. This bulge may not coincide with the normal corneal apex and is usually in the lower or to the side of the center of the cornea. In such cases, the astigmatism is known as irregular because it does not have a pattern. Special contact lenses will correct the irregular astigmatism. Eyeglasses tend not to correct such astigmatism. 

 

Today, most contact lens specialists will fit you in RGP Scleral lenses. These lenses are large and never touch the cornea but only the sclera. When inserting these lenses, the reservoir is filled with saline solution, which in essence corrects the irregular astigmatism by masking it with fluid and the back surface of the scleral lens.

Specialty Fits for Keratoconus

Depois de determinarmos que você tem um alto nível de astigmatismo, examinaremos sua córnea com um biomicroscópio para determinar se podemos visualizar uma anomalia ou protuberância em seu olho.

O achado mais comum é um pigmento periférico chamado anéis de Fleisher, que são anéis pigmentados na borda da córnea. Esses anéis só aparecem no Ceratocone. Então, uma topografia do seu olho é tirada. Veja a imagem anterior.

Observe uma elevação inferior em vermelho. A topografia mostra a elevação, geralmente na parte inferior do olho, embora também possa ser nas laterais. Também estamos observando a espessura da sua córnea.

Nosso especialista medirá então o diâmetro da sua córnea e o tamanho usual da pupila em iluminação regular. Então, uma lente escleral de material RGP será colocada no seu olho. Essas lentes são grandes em diâmetro, abobadadas e sempre repousam no branco dos seus olhos (esclera) fora da área da córnea. A lente é preenchida com uma solução salina não preservada; a seção abobadada permanecerá com esse fluido durante o uso da lente. Essa solução é rotineiramente substituída diariamente para evitar contaminação. O resultado é uma superfície refrativa na qual o formato irregular da sua córnea é substituído por uma curvatura suave que deve tornar o astigmatismo da córnea irrelevante, exigindo apenas a correção refrativa do interior do seu olho. Sua visão deve ser muito melhor, pois agora corrige a quantidade total do seu astigmatismo da córnea, que os óculos de grau geralmente não conseguem corrigir.

Se considerarmos que seu astigmatismo irregular está aumentando, podemos optar por fazer um procedimento chamado cross-linking. O objetivo é tentar interromper o aumento do astigmatismo irregular da córnea. O cross-linking não é uma correção refrativa. Ele torna o tecido da córnea mais forte. O cross-linking é um procedimento relativamente novo no qual a córnea é exposta à luz UV após a remoção leve do epitélio da córnea. Então, a riboflavina (vitamina B) é instilada no olho por cerca de 20-30 minutos. O olho é então exposto à luz UV por 30 minutos. Este processo torna a córnea (mais rígida) ao unir as fibras de colágeno. O médico prescreverá um antibiótico que geralmente tem cortisona. A segurança é de cerca de 95%. e os resultados finais são encorajadores, pois o astigmatismo permanece constante, permitindo que você continue usando suas lentes de contato e evitando o transplante de córnea.
